JKN Strengthens Indonesia's Human Capital as BPJS Kesehatan Expands Healthcare Access and Sustainability
Jakarta. Indonesia's National Health Insurance (JKN) program is reinforcing its role as one of the country's most important investments in human capital, combining broader access to quality healthcare with sound financial management and digital innovation to support a healthier, more productive workforce.
During its 2025 Public Expose on Program Management and Financial Performance, BPJS Kesehatan highlighted the continued growth of the JKN program, demonstrating how universal health coverage is contributing not only to better health outcomes but also to Indonesia's long-term economic competitiveness and its vision of achieving Golden Indonesia 2045.
"JKN is far more than a healthcare financing program," said BPJS Kesehatan President Director Prihati Pujowaskito. "It is a foundation for building a healthier, more productive and globally competitive workforce. When people have access to quality healthcare without facing financial hardship, they can continue working, innovating, and contributing to national development."
Nearly Universal Health Coverage
As of Dec. 31, 2025, JKN covered 282.7 million people, equivalent to 98.62% of Indonesia's population, making it one of the world's largest single-payer health insurance programs.
Growing participation has been matched by rising utilization. Throughout 2025, JKN financed more than 725.3 million healthcare services, averaging over 1.9 million healthcare visits every day.
To meet increasing demand, BPJS Kesehatan has continued expanding its nationwide healthcare network through partnerships with 23,770 primary healthcare facilities, 3,194 referral hospitals, and 6,190 supporting healthcare providers across Indonesia.
Digital transformation has also become a cornerstone of service delivery. Participants now enjoy easier access to administrative services and healthcare information through the Mobile JKN application, the PANDAWA WhatsApp service, and Care Center 165, enabling faster and more convenient access to healthcare services.
According to Prihati, the success of JKN is underpinned by prudent financial management and strong governance.
At the end of 2025, the Health Social Security Fund (DJS Kesehatan) recorded net assets of Rp30.04 trillion, sufficient to cover an estimated 1.88 months of healthcare claims, in line with regulatory requirements.
Meanwhile, investment returns reached Rp3.94 trillion, reflecting BPJS Kesehatan's commitment to careful and sustainable fund management.
The organization also maintained a strong governance record by receiving an Unqualified Opinion (Wajar Tanpa Modifikasi/WTM) from independent auditors for the 12th consecutive year, extending a track record of 34 consecutive unqualified audits dating back to its predecessor, PT Askes (Persero).
Additional governance achievements included:
- Organizational Governance Score: 97.67
- Governance, Risk and Compliance (GRC) Maturity Score: 4.01
- Baldrige Excellence Framework Score: 685
- Corruption Eradication Commission (KPK) Integrity Survey Score: 80.48
"These achievements demonstrate our commitment to transparency, accountability and responsible management of public funds," Prihati said.
Chairman of the Supervisory Board Stevanus Adrianto Passat added that safeguarding contributions made by participants, employers and the government requires consistent implementation of good governance principles.
"Maintaining JKN's long-term financial sustainability, improving service quality, expanding active membership and strengthening collaboration with all stakeholders remain our shared priorities," he said.
Driving Economic Growth Beyond Healthcare
The impact of JKN extends well beyond the healthcare sector.
Research conducted by the Institute for Economic and Social Research (LPEM FEB UI) estimates that the program contributes approximately Rp129 trillion to Indonesia's gross domestic product while supporting around 3.5 million jobs through multiplier effects across healthcare services, food and beverage industries and social services.
The study also found that JKN helped lift 8.1 million Indonesians out of poverty between 2018 and 2019 while protecting another 16 million people from falling into poverty because of catastrophic medical expenses.
Researchers further estimated that every 1% increase in JKN participation raises household per capita spending by 2.71% while contributing to an increase in life expectancy of up to three years.
"These findings reinforce that JKN is not only a social protection program but also an important investment in Indonesia's long-term productivity and economic development," Prihati said.
Preparing for the Future
As healthcare demand continues to grow, BPJS Kesehatan recognizes that maintaining the program's long-term sustainability requires continuous innovation and collaboration.
Healthcare claims reached Rp191.3 trillion in 2025, with 26.42% allocated to catastrophic diseases—many of which can be prevented through healthier lifestyles and early detection.
To address these challenges, BPJS Kesehatan is strengthening preventive healthcare initiatives, promoting healthy living, improving service quality, enhancing premium collection and reinforcing cost-control measures.
Healthcare experts welcomed the program's continued progress.
Timboel Siregar, Advocacy Coordinator at BPJS Watch, described JKN as a key constitutional mandate that has steadily improved healthcare access, service quality and governance through sustained collaboration among stakeholders.
Meanwhile, Professor Telisa Aulia Falianty of the Faculty of Economics and Business at the University of Indonesia said resilient healthcare financing should be viewed as a long-term investment in human capital rather than merely a public expenditure.
She emphasized that strengthening JKN's sustainability will require financing reforms based on mutual cooperation, greater efficiency across the healthcare system and stronger collaboration among government institutions, healthcare providers, businesses and the public.
Looking ahead, BPJS Kesehatan reaffirmed its commitment to working closely with central and regional governments, healthcare providers, employers and other stakeholders to ensure JKN remains financially sustainable while continuing to improve the health and productivity of Indonesians.
With nearly universal coverage, robust governance and growing economic impact, JKN continues to serve as a cornerstone of Indonesia's ambition to build a healthier, more resilient and globally competitive nation on its journey toward Golden Indonesia 2045.
