BPJS Kesehatan Says JKN Powers Health, Productivity and Economic Growth

Jakarta. BPJS Kesehatan says Indonesia's National Health Insurance (JKN) program has evolved into one of the country's most important investments in human capital, expanding access to healthcare while supporting long-term economic productivity and the sustainability of the national health system.

Presenting its 2025 Program Management and Financial Report during a public expose in Jakarta on Thursday, BPJS Kesehatan highlighted record participation, growing healthcare utilization and continued financial resilience, underscoring the program's role in supporting Indonesia's vision of becoming a developed economy by 2045.

"Our mission goes beyond financing healthcare," said BPJS Kesehatan President Director Prihati Pujowaskito. "JKN is building a healthier, more productive and more competitive workforce. When people can access quality healthcare without worrying about financial hardship, they are able to work, innovate and contribute more to national development."

By the end of 2025, the JKN program covered 282.7 million people, equivalent to 98.62% of Indonesia's population, making it one of the world's largest single-payer health insurance systems.

The expanding membership has been accompanied by rising demand for healthcare services. Throughout 2025, JKN financed more than 725.3 million healthcare visits, averaging over 1.9 million services every day.

To meet growing demand, BPJS Kesehatan has continued expanding its nationwide healthcare network, partnering with 23,770 primary healthcare facilities, 3,194 referral hospitals, and 6,190 supporting healthcare providers across the country.

Digital innovation has also become a key pillar of service delivery. Participants can now access healthcare information and administrative services through the Mobile JKN application, the PANDAWA WhatsApp service, and the Care Center 165, reducing waiting times and improving convenience.

Strong Governance Supports Sustainability
BPJS Kesehatan says maintaining public trust requires not only broad coverage but also transparent and accountable management.

Chairman of the Supervisory Board Stevanus Adrianto Passat said safeguarding public funds contributed by participants, employers and the government remains a top priority.

"Maintaining the long-term financial sustainability of JKN, improving service quality, expanding active membership and strengthening collaboration with stakeholders will remain our collective challenge," Stevanus said.

The institution reported that the Health Social Security Fund (DJS Kesehatan) remained financially sound at the end of 2025, with net assets of Rp30.04 trillion, equivalent to 1.88 months of projected claim payments, in line with regulatory requirements.

Meanwhile, investment returns reached Rp3.94 trillion, reflecting what BPJS Kesehatan described as prudent and sustainable fund management.

The organization also maintained a strong governance record, receiving an Unqualified Opinion (Wajar Tanpa Modifikasi/WTM) from independent auditors for the 12th consecutive year, extending a track record that dates back 34 consecutive audits, including the period when the institution operated as PT Askes (Persero).

Additional governance indicators also remained high, including:

  • Organizational Governance Score: 97.67
  • Governance, Risk and Compliance (GRC) Maturity Score: 4.01
  • Baldrige Excellence Framework (BEF): 685
  • Corruption Eradication Commission (KPK) Integrity Survey Score: 80.48

A Driver of Economic Growth
Beyond healthcare, BPJS Kesehatan says JKN has become a significant contributor to Indonesia's broader economy.

According to research by the Institute for Economic and Social Research, Faculty of Economics and Business, University of Indonesia (LPEM FEB UI), the program contributes approximately Rp129 trillion to Indonesia's gross domestic product while supporting around 3.5 million jobs through multiplier effects across healthcare services, food and beverage industries, and social services.

The study also found that JKN helped lift 8.1 million Indonesians out of poverty during 2018–2019 and protected another 16 million people from falling into poverty due to catastrophic medical expenses.

Researchers estimated that every 1% increase in JKN participation raises per capita household spending by 2.71% while increasing life expectancy by as much as three years.

Preparing for Future Challenges
As healthcare utilization continues to rise, BPJS Kesehatan acknowledges that maintaining financial sustainability will require continuous reforms.

Healthcare claims reached Rp191.3 trillion in 2025, with 26.42% allocated to catastrophic diseases, many of which can be prevented through healthier lifestyles and earlier diagnosis.

To address these pressures, BPJS Kesehatan is expanding preventive healthcare programs, promoting healthier lifestyles, improving service quality, strengthening premium collection and enhancing cost-control measures.

Healthcare policy experts welcomed the program's progress while stressing the importance of continued collaboration.

Timboel Siregar, advocacy coordinator at BPJS Watch, said the JKN program represents the constitutional mandate to provide healthcare for all Indonesians and has shown measurable improvements in service quality and governance.

Meanwhile, Telisa Aulia Falianty, professor at the Faculty of Economics and Business, University of Indonesia, described sustainable healthcare financing as a long-term investment in human capital and national competitiveness.

She said strengthening JKN's financial resilience will require continued reforms based on mutual cooperation, greater efficiency across the healthcare system and closer collaboration among government agencies, healthcare providers, businesses and the public.

BPJS Kesehatan reaffirmed its commitment to working with central and regional governments, healthcare providers, employers and other stakeholders to ensure the long-term sustainability of JKN and support Indonesia's ambition of building a healthier, more productive workforce on the path toward Golden Indonesia 2045.
