BPJS Kesehatan Clarifies Control Letter Requirements for JKN Patients

Jakarta. Certainty over treatment schedules remains one of the most common concerns among National Health Insurance (JKN) participants when accessing hospital services, including follow-up consultations. BPJS Kesehatan said participants who need to attend a follow-up appointment with a doctor are required to bring a control letter as part of the administrative requirements.

BPJS Kesehatan Head of Public Relations Rizzky Anugerah said the control letter is issued by a healthcare facility based on the recommendation of the physician in charge of the patient. The document contains the schedule for the next follow-up visit and is intended to ensure continuity of care according to the patient's medical needs.

“The purpose of issuing a control letter is to provide certainty over service schedules for JKN participants who require follow-up consultations, either after hospitalization or based on a doctor's assessment following outpatient treatment. With a clear schedule, participants can receive services in line with the treatment plan established by their physician,” Rizzky said.

He explained that there are two conditions under which a control letter may be issued. First, participants who have completed inpatient treatment but still require further monitoring will receive a control letter for outpatient services according to a schedule determined by their doctor. Second, participants undergoing outpatient treatment who their doctor assesses as requiring further follow-up care will also receive a control letter specifying the next appointment date.

“The control schedule stated in the letter is a follow-up to the doctor's examination and medical assessment. Therefore, schedules may differ from one participant to another depending on their health condition. During outpatient care, the doctor will determine whether the participant still requires follow-up consultations or has completed treatment. Likewise, patients discharged from inpatient care will receive guidance on the next steps in their treatment, including follow-up appointments if additional monitoring is needed,” Rizzky explained.

He added that a control letter is valid for a single visit. If, after examination, the doctor determines that additional follow-up care is necessary, a new control letter may be issued based on the participant's medical needs and the validity period of the referral they hold.

“A control letter should be issued before the follow-up service is provided, as part of efforts to maintain continuity of care and ensure service certainty for patients. After receiving treatment from a doctor, the next follow-up appointment can be scheduled as early as the following day, depending on the recommendation of the physician in charge,” Rizzky said.

Participants are required to attend their appointments on the scheduled date. However, if they need to reschedule, they may coordinate with healthcare facility staff while taking into account the availability of the physician in charge.

“Having a confirmed follow-up schedule not only makes it easier for participants to access continued care, but also supports the continuity of treatment and monitoring of their health condition. At the same time, hospitals can manage service capacity more effectively, allowing healthcare services to be delivered more optimally,” Rizzky said.

Rizzky emphasized that follow-up schedules are determined based on the medical judgment of the physician responsible for the participant's care, rather than solely on administrative considerations. The objective is to ensure participants receive continuous, timely, and medically appropriate healthcare services.
