Jakarta. State health insurer BPJS Kesehatan recently promoted long-term care innovation at the 2025 World Social Security Forum (WSSF) in Kuala Lumpur.

In Indonesia, the national health insurance scheme is better known as JKN, short for Jaminan Kesehatan Nasional. JKN, which has covered over 282 million people, is the world’s largest health insurance scheme, according to BPJS Kesehatan. The insurer said that it had made efforts to make sure that JKN participants were protected into old age. To this end, BPJS Kesehatan is strengthening its long-term care services and also carries out digital transformation of health insurance.

Ghufron Mukti, the president director of BPJS Kesehatan and chair of the Technical Commission on Medical Care and Sickness Insurance, pointed out the growing elderly population in Indonesia. More people have also been getting chronic diseases, thus sparking the need for long-term care.

“Demographic challenges and the burden of long-term healthcare costs demand a new approach through prevention, early detection, strengthening primary care, and digital transformation," Ghufron said.

BPJS Kesehatan has developed various innovations, such as Health History Screening as a preventive and promotive effort, including for elderly participants. JKN participants now have easier access to services at health facilities, simply by providing the national identification number (NIK) on their ID card or family card.

BPJS Kesehatan also supports big data analytics to map chronic disease trends as well as the service needs for elderly participants. These analyses can help with the policy formulation processes.

"This year, the trend of increasing cases of diseases with catastrophic costs continues to rise.  Our data shows that as of August 2025, the highest number of cases of diseases with catastrophic costs was heart disease. About 10.96 million cases were covered by the JKN,” Ghufron said.

Kidney failure is the second-highest number of diseases with high medical costs, with 7.32 million cases, followed by stroke (4.03 million cases) and cancer (3.54 million cases).

BPJS Kesehatan has undertaken various efforts to promote JKN literacy among the public, including via official digital channels. The insurer has also partnered with local governments and communities to promote JKN.

"We hope this forum will provide a platform for discussion, exchange ideas, strengthen international networks, and formulate concrete steps with other countries to realize a long-term, inclusive, and sustainable health insurance system," Ghufron said.
